Download and complete the CONTRACTING PROFILE document. Return the contracting profile as ONE PDF  to [email protected],  please do not send individual pages. 

Required documents :

• Copy of your life insurance  license
• Copy of your driver's license 

• Voided check or approved EFT form, see page 1 of contracting packet for more detail
• RegED Anti-Money Laundering (AML) certificate
• ​Errors & Omissions Insurance (E&O) 

SCANNING APP RECOMMENDATION  We use Adobe Scan from our smart phones to scan all of our application and documents.  It is user friendly and free to use.  Download from your app store.

You may write business with all carriers but they will only process your contracting upon submission of your first applications depending on PRE-APPOINTED STATES.

​AML Training with RegEd

Equis Financial REQUIRES the RegED Anti Money Laundering course.   No other AML will be accepted even if obtained recently. The course is free with this link.

Select  Opt-out of CE credits option, this AML Course cannot be applied towards CE credits. Upon completing the course take a screenshot of completion page and attach to your contracting packet. 

Errors & Omissions Insurance (E&O)  

This insurance helps to protect against claims arising from the sale and servicing of insurance products.  E&O is not required to contract with most of our carriers but is highly recommended.

We recommend using the NAPA, National Association of Professional Agents.  You can access their information HERE and clicking on the Life & Health Agent E&O button.  

For any of the annuity carriers your E&O also needs additional coverage for fixed/indexed annuities.

What Happens NEXT? 

Surancebay e-mails

Surancebay is the online system used to securely transmit your contracting info to each carrier. After your Contracting Packet is APPROVED, you will receive individual emails from Surancebay to confirm each carrier contact. You must open and confirm each email as they will each be carrier specific.  

PLEASE NOTE: The GPM contracting link will come from the Equis Financial, usually sent 2-3 days after you receive the Surancebay emails.  GPM requires a GPM writing number before writing your first GPM app, it can take up to 3 weeks to receive your writing number. Don't delay this step, GPM is a frequently used carrier.  If you don't receive the GPM email from Equis Financial, use the chat feature from your Equis dashboard to request the GPM contracting email.
